In surgical treatment, intramedullary nail fixation has been recommended [2], and plates were previously chosen as the fixation device owing to the technical impracticability of nailing due to bowing of the femur or cortex thickening [7].
Various internal fixation devices have been used to treat femoral intertrochanteric fractures, including sliding hip screws (SHS), percutaneous compression plate (PCCP), gamma nail (GN), proximal femora nail (PFN), proximal femoral nail antirotation (PFNA), Proximal Femoral Nail Antirotation-I I (PFNA-II), and the InterTan intramedullary nail. Compared to other implants, the advantages of InterTan nail include the ability to maintain compression, eliminate Z-effect intertrochanteric rotational stability and medial migration, control rotation during reduction, and prevent periprosthetic fractures.

For the surgeon, the tricky part of the process is placing the screws accurately so that they line up exactly with the predrilled holes of the intramedullary nail. Traditionally, surgeons drill freehand, through the bone's cortex and into the predrilled hole of the implant.
