Form 11-C(redirected from Forms 11-C)
A form the owner of a gambling establishment must file if he/she hires agents who take bets on behalf of the establishment. On Form 11-C, the owner must list the names, addresses, and employer identification numbers of these agents. For example, if Tom is a bookie and has six employees who take bets for his business, he must list all six on Form 11-C. However, he does not have to list his other employees who do not take bets. The purpose of the form is to ensure that all bet takers pay the appropriate occupational tax.
