A form that one uses to amend a previously filed Form 1040, which is used to report one's income and pay the income tax.

If the home is purchased between April 16, 2009, and November 30, 2009, the taxpayer can still claim the credit on a 2008 tax return by requesting an extension of time to file (Form 4868) before the April 15 deadline or by filing an amended return (Form 1040-X).
