Turnaround Document

Turnaround Document
A document resulting from a computer output to which information is added. The information is then fed back into the computer. A common example is a utility meter reading.

My thought would be to simply print out a turnaround document at the end of your register tape and ask the customer to fill it out and leave it with the cashier or the courtesy booth, or to go online and complete the information there.
Many of the institutions that have successful implementations use a turnaround document.
