Order ticket
A form detailing an order instruction that a customer gives an account executive.

Ticket
Informal for the form on which an investor makes an order to a broker. It is not always necessary to fill out a ticket, as some orders can be made over the phone or via the Internet. It is also called an order ticket.

A paper form on which a security order is written. Also called ticket.
