Form PD 4632
A registration form one must file to be eligible to purchase U.S. Treasury bills (which have maturities under one year) at auction. In filling out the form, one must specify the specific maturity one wishes to buy. Forms may be filed at any Federal Reserve Bank or any of its branches.

Form PD 4632
A Federal Reserve form that investors must complete in order to buy Treasury bills at auction from a Federal Reserve bank. The form may be submitted by mail or in person to any of the 12 Federal Reserve banks or any of the 25 branch banks. The form has three categories, depending on whether a 13-week, 26-week, or 52-week bill is sought.
