Affidavit of Performance

An affidavit submitted by a broadcaster to an advertiser stating that a commercial or other advertisement has been aired according to the specifications set by the advertiser. The affidavit of performance states the services rendered and the amount the advertiser owes. An affidavit of performance is notarized and is made under penalty of perjury.
